OP-KISS module: List of indicator procedures

Indicator procedures

Participating departments can select one or more indicator procedures for surveillance. Indicator procedures are defined by procedure codes (OPS codes) and ICD diagnosis codes, if applicable. All listed codes of a respective indicator procedures must be included if a department decides to include it in their surveillance. Recording must not be limited to individual codes within the indicator procedures, otherwise this would reduce comparability between departments.

For each indicator procedure, it is specified whether the surveillance period for surgical site infections is 30 or 90 days. If a reoperation is performed in the same surgical area before this period expires, surveillance ends on the day of the reoperation. The list of indicator procedures and the codes to be included within the respective indicator procedures is updated annually by the NRZ.

Main procedure

An operation can have several procedure codes, a main procedure and possibly several secondary procedures. The surgeon determines which is the main procedure for each operation. Only those operations for which the main procedure, or the leading procedure in terms of the surgery, corresponds to an OP-KISS indicator procedure, are included in OP-KISS surveillance. For some indicator procedures (e.g. COBY), potential secondary procedures must also be considered.

Revision surgeries

Revisions or second surgeries in the same surgical area within 30 or 90 days (depending on the indicator procedure) after an operation are not counted as indicator procedures. Normally, revisions receive different procedure codes than the original surgery anyway. However, for some types of surgery, revision surgeries may receive the same code as the initial surgery (e.g. COLO, GC). These revisions may not be included again in the OP-KISS recording if the previous procedure was less than 30 or 90 days ago.

Multiple procedures

In the case of extensive operations, several procedures may be performed that could be an indicator procedure (e.g. extensive abdominal operations on the colon with removal of the gall bladder and appendix). For OP-KISS, only the type of procedure that corresponds to the main procedure is considered.

Procedure codes of indicator procedures

The accompanying documents specify the codes to be included in the indicator procedures, providing as many digits as needed to clearly identify them. Codes with more digits than are displayed are generally included: Example: The specification 5-511.0 includes the codes 5-511.00, 5-511.01 and 5-511.02, but not 5-511.3. The placeholder “_” stands for exactly one character at this position in the code. Example: The specification 5-811._h includes the codes 5-811.0h, 5-811.1h, etc.; but not 5-811.y (because the letter "h" does not follow in the 6th position).
